A 54-year-old business owner pleaded guilty to issuing a false certificate of insurance claiming to have workers’ compensation insurance with the New York State Insurance Fund, according to the NYSIF.
NYSIF said that authorities arrested John Cappucci, of Levittown, New York, who owns and operates a cement business, in March and charged him with issuing a forged NYSIF certificate as proof of having workers’ compensation coverage.
Investigators said Cappucci produced the certificate in March 2004 as proof of coverage to obtain work from a NYSIF-insured contractor. The contractor was left with a financial loss as a result of the fraudulent certificate.
NYSIF has an electronic certificate of insurance system. Each NYSIF certificate has a unique validation number that helps prevent fraud.
